Appointments
Appointments can be made by calling our office at 973-209-1200.
Patients are
seen by appointment only. We
suggest you call our
office approximately 1 - 3 months before your annual visit is due,
so we can schedule a time that is most
convenient
for you.
If you have a problem or an emergency, we will schedule an
appointment with Dr. Fred Nichols, Dr. Edward Zarczynski,
or Joan Weglein, RN-C, MSN as soon as possible.
If you are
unable to
keep your appointment, please notify us as
soon as possible so we can reschedule your appointment for
another time.

You do not have your copay (cash/check only - No credit/debit cards
accepted ).
Information
RequirementsWhen you visit our offices, please be sure to bring
your medical
insurance card - a copy of your insurance card is required for EVERY visit.
If you haven't visited us within the past
year, or if your
previous registration information has changed since you last
visited,
you will need to complete a new registration. Your insurance
co-payment
will be collected at check in time and please note that we only accept cash
or
checks and do not accept credit/debit cards.
Please arrive approximately 15 minutes before your
scheduled
appointment in order to complete this necessary paperwork.
Test Results
Women's Total
Health will notify patients of normal pap and
mammogram results by mail. If there is evidence of an abnormality in your pap smear or if your mammogram
results are questionable, your physician will contact you by phone to discuss a
recommended follow-up plan.
Lab and Radiology Forms Are
provided to the patient at the time of the office visit.
We will not fax replacement forms to any facility for any appointment if forms
are lost
or forgotten.
Appointment will be rescheduled.
A new form will be issued within 24 hours of phone call for patients to pick
up
from Women's Total Health.
Prescription and
Refills
Routine prescription refills will be made during
regular office hours only. To request a prescription refill, please call our
office and leave a message indicating your name, date of birth, the name of the
medication, the prescription number and the pharmacy name and phone number.
